Becoming A New Client
At Woodstock Animal Hospital, you can expect state of the art medical care for your four-legged companions. We believe in nurturing the human-animal bond and creating a harmonious relationship between people and animals.
1. Schedule An Appointment
To schedule your pet’s initial visit efficiently, please contact our office directly by phone. Alternatively, you may utilize the contact features available on our website and we will reach out with any additional questions.
2. New Client Form
To ensure a seamless experience for your first visit, we kindly request that new clients complete the form provided below. This will enable us to better understand you and your pet, and ensure that your information is accurately entered into our system prior to your appointment.
3. Send Records
The final step! Please forward your pet’s previous veterinary records to us before your first appointment. This will allow our doctor and team ample time to review your pet’s history and ensure that accurate vaccination records are entered into our system. Records can be faxed or emailed. If you prefer to bring the records in person, please arrive a few minutes early to allow our team time to review them.
Referral Coupon
Existing clients who refer new clients will receive a $10 credit to their account for each referral.
Please ensure that you mention your referring friends name at your appointment so that we can apply the credit to their account.
